How do we schedule appointments for MDVIP patients?
We strive to give our patients the best medical care and services possible. In doing so, we would like to provide you with some information about our office and some of our policies. Your MDVIP membership includes the testing appointment and your consultation appointment (test results/care plan). The testing and consultation appointments are 60-90 minutes each. Your testing appointment will be done a few weeks before your Consultation appointment to ensure your results are complete.
New Patients will have a New Patient appointment before testing and consultation to ready all your information in your new chart at Dr. Orszulak’s office.
All other visits, follow up appointments, Health Insurance Annual Visit and sick appointments will be scheduled and billed to Insurance as usual. Co Payments and Deductibles still apply to these visits, per your Insurance company contract.
If you are hospitalized for any reason, please notify our office as soon as possible.
Maureen Westgarth Scheduling:
MDVIP testing is generally done in the mornings due to the fasting for labs. Each Morning there is a 7:00, 8:30 and 10:00. The testing should take about 90 minutes total. Maureen is also available for Sick Visits, follow up visits, Televisits and Medical Clearances throughout the day.
Dr. Orszulak Scheduling:
We schedule “Meet and Greet” appointments, Health Insurance Annual Visits, Sick Visits, follow up visits, Televisits and Medical Clearances throughout the day.
What to Bring to Your Appointment:
Bring all medications, including vitamins and supplements in a ziploc bag or update the list that was provided at your last visit with any changes or additions. Your current Health Insurance Card, Drivers License and a copy of your Health care proxy or Molst form (if you have them)
After Hours/Weekend Calls:
Call the office number (716-236-8960) as usual, the phone call will be forward and answered by either Dr. Orszulak or Maureen. If they are not available at that moment, you can leave a message and you will get a prompt call back.
