How do we schedule appointments?

We strive to give our patients the best medical care and services possible. In doing so, we would like to provide you with some information about our office and some of our policies.

If you are hospitalized for any reason, please notify our office as soon as possible.

Patient Appointments Available: Monday – Thursday 7:00-11:40 am and 1:00-4:40 pm

Dr. Orszulak Scheduling: We schedule Annual Visits with Doctor; Sick Visits and some follow up visits are with Nurse Practitioner; About 80% of her day is available for sick visits. If you are sick you must be seen for treatment. All visits are by appointment only.

Established Patients: Will be seen for a complete physical examination once a year, below is a chart of the interval at which established patients should be seen;

Insurance: It is the patient’s responsibility to understand what their insurance will pay for services, testing, copayments and vaccinations.

What to Bring to Your Appointment: Bring all medications, including vitamins and supplements in a Ziploc bag or update the list that was provided at your last visit with any changes or additions.

After Hours/Weekend Calls: Call the office number as usual. There is an answering service that will answer your calls, take a message and notify the Doctor on call. Narcotic medications will not be prescribed; Antibiotics may not be prescribed if you have not been seen in the office. If Antibiotics are prescribed a follow up appointment will be made at the time the antibiotics are prescribed; Failure to keep this appointment will result in a discharge from practice or a missed appointment fee of $50.00.

Prescriptions: All messages left on the office refill line will be answered hourly. Check with your pharmacy at the end of the day for your refill. Any medications that require a written prescription will generally not be given without an appointment. Doctor may not fill written scripts in the event a patient fails to keep a scheduled appointment.

Generally antibiotics are not given over the telephone.

Any medications that require a written prescription will take 24-48 hours to process and are available for pick up during office hours.

Payment: Copayments, Deductibles and account balances are due at the time of service and will be collected at sign in. If you do not have your copayment or account balance, you may be asked to reschedule your appointment.

Additional Fees: Disability, insurance and other forms are priced per sheet. Missed appointments without proper 24 hour notification are subject to a $50.00 fee. The fee for checks with non sufficient funds is $50.00.

Missed/Rescheduled Appointment: Two missed appointments without proper notification will be reason for dismissal from practice; chronic rescheduled appointments (3 or more) will be reason for dismissal from practice.
